Abstract

Abstract: Peripartum cardiomyopathy is a rare form of heart failure that occurs in late pregnancy or during the postpartum period, characterized by left ventricular systolic dysfunction with an ejection fraction below 45%, in the absence of other identifiable causes. This study presents a retrospective and descriptive case report of a 22-year-old woman, four days postpartum after cesarean delivery, with no known risk factors, who developed asthenia, progressive dyspnea, and palpitations. The patient was admitted to a high-complexity hospital with acute respiratory failure, requiring orotracheal intubation and vasopressor support. Initial investigations ruled out pulmonary thromboembolism and other causes of heart failure. Echocardiography revealed dilated cardiomyopathy with an ejection fraction of 28%, consistent with peripartum cardiomyopathy. This case highlights the importance of early recognition and intensive management of this potentially fatal condition, even in patients without classic predisposing factors.
