Abstract

Endomyocardial fibrosis is a rare restrictive cardiomyopathy with a not fully understood etiology, predominantly found in tropical and low-income countries. The disease is characterized by endocardial fibrosis and involvement of the heart valves, leading to reduced ventricular compliance, heart failure, and diastolic dysfunction. Its clinical presentation is progressive, with manifestations such as dyspnea, peripheral edema, ascites, and anasarca, especially in advanced stages. Diagnosis is primarily based on echocardiography, with evidence of apical obliteration and typical hemodynamic alterations. Treatment is mainly clinical, focusing on heart failure symptom control and anticoagulation, and may involve surgery in advanced cases.
